States of Licensure

Dr. Hirsh is currently licensed in New Jersey, Pennsylvania, and California.

Insurance / Payment

We are out-of-network for all medical insurance companies. If you have out-of-network mental health benefits, we will provide a detailed receipt (superbill) that you may submit to your insurance carrier for potential reimbursement. Reimbursement is not guaranteed and varies by plan, typically ranging between 50-90%. It is your responsibility to contact your insurance provider in advance to inquire about out-of-network benefits, reimbursement rates, and coverage for virtual appointments.


All services must be paid in full at the time of each session via cash, personal checks, or major credit cards. You are responsible for submitting your own claims if you choose to seek reimbursement. Additionally, we do not accept Medicare. Individuals with Medicare agree not to submit receipts for visits to Medicare, as doing so would constitute fraud.


By not accepting insurance, we are able to dedicate more time and attention to each patient, providing highly personalized and comprehensive psychiatric care without the limitations imposed by insurance companies. This approach ensures that treatment is tailored to your unique needs rather than dictated by insurance-driven restrictions. By operating independently, we can focus entirely on delivering the best possible treatment without external constraints. Please note: The initial evaluation does not initiate a professional relationship; both parties must agree to work together after the initial evaluation for a professional relationship to be initiated.

Controlled Medication Prescribing Policy

We follow a strict and responsible approach to prescribing controlled medications, and there is no guarantee that a prescription will be issued, regardless of prior use or past diagnoses. Even if a physician determines a controlled medication may be appropriate, it is typically not prescribed until a follow-up visit to allow for thorough record reviews and necessary assessments. We do not accommodate urgent requests for controlled medications, and patients seeking immediate prescriptions should consider alternative physicians. Those with relevant medical or pharmacy records should bring them to their initial appointment for review. Finally, disagreement with a diagnosis or a decision not to prescribe does not warrant a refund or waiver of the appointment fee.


Disability Assessments

At this time, Dr. Hirsh does not conduct long-term disability evaluations or assessments for legal purposes. Requests for short-term leave may be considered on a case-by-case basis for established patients who have been in regular treatment for at least six months. Dr. Hirsh also does not provide documentation or evaluations for emotional support animals or service animals.

Cancellation Policy

We require a minimum of 48 hours notice for cancellation of your appointment.
